Sorry you got ignored, there is just so much spam in the guest section (you would probably do better by registering).
They mostly push down at the front-middle, but it depends on size and placement. They're not really great for stopping sound- even with a perfect gag, one can make noises in their throat. But it'll definitely stop anything coherent.
