I did a quick google but didn't find a damn thing. Also ebay came up with zero.
You might end up having to go through a private collector. I would say look into some time period based torture device forums or something of that nature.

Hello everyone. I have been watching this forum for quite some time and finally decided to sign up and participate. So I’m just reading through the threads and thought I might be able to help a little on this one.
I seen a show on the history channel about torture devices a while back, and I remember this one was mentioned. A quick Google comes up with a bunch of pages with the same 3 or 4 sentence explanation of its medieval torments.
However I dug a little deeper and found “The Pear†goes by a few names….

I would avoid the authentic ones since they seem to be incredibly rare and expensive. Not to mention the authentic ones have barbs on them designed to intentionally tear and rip into body cavities. I would recommend a medical grade speculum or anal spreader. It would do essentially the same thing, much cheaper, and without all the blood and pain of ruptured entrails (unless you are going for that, then more power to you, and please post pics!!)
However, since you seem as though you REALLY NEED one, you can obtain one of these specifically made for bondage at this underground and kinda seedy looking distributor here http://www.verydirtytoys.com/PearofAnguish.html I would also think you could probably find a decent replica at a Renaissance Fair maybe?
